Clinical Process Mapping Session

This presentation includes discussion on the participation of the Gerald Ignace Health Clinic in the Innovations in Planned Care for the Indian Health System Collaborative--coordinated by the Chronic Care Initiative for the Indian Health Service. Also, gives an overview of the Care and the Improvement Models and  their use for rapid cycle changes and improvement facilitation.

Eligibility and Documentation of Eligibility

This very informative presentation discusses information on eligibility, documentation of eligibility, and the latest issues regarding eligibility. The presenatation concluded with a question-and-answer period in which Urban Program Directors could voice their concerns and ask questions relating to eligibility, including the lack of consistency in contracts from program to program, consistency with regards to Public Law, and the guidelines for documentation of eligibility.

Resource and Patient Management System (RPMS)
